US Club Soccer Northeast Pre-Academy League

GAME REPORT:

Head coaches of the home team is to bring a blank copy of the Northeast Pre-Academy League Game Report to each game.

The referee is responsible for returning the completed Northeast Pre-Academy League Game Report to US Club Soccer within 24 hours of the conclusion of the match.

The form can be returned via e-mail to Operations Manager Daniel Allen at dallen@usclubsoccer.org or via fax at (843) 614-4146.

GAME CHANGE REQUEST:

[Game Change Request Form]

Schedule changes should be avoided. However, in the event of an emergency, any and all requests for schedule changes must be approved by US Club Soccer. Requests must be submitted at least 28 days in advance of the currently-scheduled game via the Game Change Request Form.

The form should be returned to Operations Manager Daniel Allen at dallen@usclubsoccer.org.

RECOMMENDED PLAYERS:

[Recommended Players Form]

After each game, each team is responsible for providing two recommended players from the opposing team for consideration in player identification camps via the Recommended Players Form.

The form should be returned to Operations Manager Daniel Allen at dallen@usclubsoccer.org within 24 hours of completion of the game.
